Palliative Care APP (Full Time)

Provide initial and ongoing clinical assessment for patients with advanced or progressive medical conditions across inpatient, outpatient, and virtual settings. Modify the plan of care as needed in collaboration with attending physicians, focusing on symptom management, decision-making assistance, and goals of care. Facilitate emotional, spiritual, and bereavement support while coordinating care until death or discharge. Serve as a clinician educator to support multiple palliative care educational endeavors throughout the South Shore community. This full-time, day/eve rotating role is part of an established, growing service dedicated to developing individualized treatment plans for patients and families.
