Job Summary
Licensed nursing professional providing palliative care support and administrative coordination: assist Palliative Care Managers, coordinate referrals and care for current patients and those discharging from inpatient facilities, triage urgent issues, manage physician orders, and support clinical staff with documentation and care plans. Responsibilities include telephone triage, coordinating with care teams, reviewing clinical documentation, handling after-hours call reports, and contributing to special projects (quality management, utilization review). The role requires nursing education, strong communication, and coordination skills, with preferred bilingual (English/Spanish) proficiency. Required Qualifications
- Graduate of a school of Nursing
- National Provider Identifier (NPI) number
- BLS - American Heart Association certification (Vendor Managed)
Desired Qualifications

- 1 year experience in home health, hospice or long-term care setting preferred
