PAL Facilitator/FYI Case Manager

Job Summary
Create self-sufficiency plans for foster care youth and alumni, including education, vocational training, and living skills. Review, evaluate, and update individualized service plans, case notes, and financial distributions. Assist participants with accessing community resources such as housing, program enrollments, and employment services. Develop professional relationships with agencies to coordinate services and deliver independent living skills curriculum. Travel throughout the multi-county region to provide client services and enhance program delivery. Complete TWC grant documentation accurately and stay current on TDFPS services to help youth with job searches and online profiles. Liaise with community agencies and host quarterly meetings with TWC representatives.
Required Qualifications
- Bachelor's Degree in a related field
- Minimum 1-year prior related casework experience
- Proficient knowledge of and/or work experience using community resources
- Ability to speak, read and write English
- Ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable
- Must provide proof of liability insurance
- Must be eligible to be insured under Buckner's insurance policy
- Must be age 21 or older to drive on behalf of Buckner
- Ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people
- Ability to walk, stand and sit, sometimes for prolonged periods of time
- Must be available for travel as needed throughout a multi-county region
Desired Qualifications
- Ability to speak, read, and write Spanish
- Prior related experience in delivering services to youth and/or young adults
- Experience in the delivery of vocational or other self-sufficiency services
