Job Summary
Support and accompany clients in their own living environment, structuring daily life through household management and appointment organization. Foster independence, social participation, and active social integration by strengthening individual resources and autonomy. Collaborate with statutory guardians, doctors, therapists, and payers to facilitate attendance at administrative appointments and institutions. Conduct professional work based on individual help plans (BEI-NRW), including documentation, reporting, and crisis intervention within the team. Engage in network work and cooperation to maintain social contacts while contributing to the help plan process. This role offers flexible working hours and a 30-32 day vacation allowance. Required Qualifications
- Abgeschlossene Ausbildung als Erzieher\*in, Heilerziehungspfleger\*in oder vergleichbar
- Abgeschlossenes Studium der Sozialen Arbeit, Sozialpädagogik oder vergleichbare Qualifikation
- Erfahrung in der Arbeit mit Menschen mit Unterstützungsbedarf
- Kenntnisse im Bereich der Teilhabeplanung (z.B BEI-NRW) oder die Bereitschaft zur intensiven Einarbeitung
- Erfahrung in Dokumentation, Berichtswesen sowie in der Zusammenarbeit mit Kostenträgern
