Job Summary
PACT Case Manager coordinates services for individuals with mental health and/or substance use needs under the PACT program, collaborating with law enforcement to divert individuals toward treatment and community services. The role involves connecting clients and families to resources, coordinating therapeutic and community-based services, and assisting with transportation as needed. The position requires a Bachelor’s degree, Colorado driver’s license, CPR/BLS certification prior to independent patient care, and strong documentation/communication skills; telehealth familiarity and experience in behavioral health case management are preferred. The incumbent may travel between Health Solutions West sites to support a diverse patient population across multiple settings.
Required Qualifications
- Bachelor’s degree required (preferably in human services)
- Bachelor’s degree in a non-related field may perform the functions of this position only if they have one year experience in a behavioral healthcare field
- Minimum of two (2) years human service related experience preferred
- Current valid Colorado Driver’s License required
- Certified Addiction Specialist strongly preferred
- CPR/BLS Certification for Healthcare Provider prior to independent patient care required
- Experience with substance abuse and mental health-related case management and treatment preferred
- Working knowledge of Windows-based applications (Word, Excel, Outlook)
