Job Summary
Provide and coordinate psychosocial services for participants and their families across the PACE Center, home, hospital, nursing home, or assisted living facility. Deliver community-based case management services while collaborating with an interdisciplinary team to determine optimal care plans. Navigate stairs and narrow spaces to physically access homes and facilities. Manage multiple priorities independently with a high degree of discretion. Monday-Friday 8:00-4:30pm. Requires a Master's degree in Social Work and Kentucky CSW or LCSW licensure.
Required Qualifications
- graduate of a master's program in Social Work accredited by the Council of Social Work Education
- licensed by the state of Kentucky as a Certified Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW)
- two years' social service experience working in a case management capacity
- at least one year of experience working with the frail and elderly
- demonstrated capabilities in networking with community resources
- knowledge of such resources
- communication skills, including face-to-face, telephone, and computer skills
- attention to detail and organization
- ability to interact successfully with participants, families, and other agencies to present a positive and professional image
- knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team
- ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
- ability to work unsupervised exercising a high degree of discretion & independent judgment
- ability to physically access any home, hospital, nursing home, and ALF (i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.)
