PACE Social Worker

Job Summary
Provide social work services to participants and families in the PACE program, including screening and assessments, case management, emotional support, counseling, support related to legal affairs, assistance in planning for caregiving and financial services. Develop and document individualized social services plans of care with the interdisciplinary team, in accordance with program policies and regulatory standards. Requires Kentucky CSW or LCSW licensure, CPR, ability to travel within the service area, and proficiency with EMR. Must communicate effectively with internal stakeholders and external partners; strong organizational skills; knowledge of healthcare systems and Medicare/Medicaid benefits and regulations.
Required Qualifications
- Licensed in Kentucky as Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW)
- CPR certification
- Minimum 1 year experience with frail or elderly population (or training upon hire)
- Travel to participant homes and other locations within service area
- Proficiency with electronic medical record (EMR)
- Ability to work independently and as part of a team
- Excellent communication and organizational skills
- Knowledge of healthcare systems, Medicare/Medicaid benefits and regulations
- Knowledge of safety and infection control in healthcare facilities
