Job Summary
Coordinate home health staff and services for ESP participants as ordered by clinical staff, adhering to individual care needs. Schedule in-home services, manage referrals, and coordinate with preferred vendors while reviewing participant lists weekly for accuracy. Distribute Community Health Aide schedules, oversee verification workflows, and document all changes in the EMR and EPIC system. Communicate effectively with staff, families, and contractors to resolve scheduling conflicts under time-limited circumstances with minimal supervision.
Required Qualifications
- High School graduate or equivalent
- Good organizational skills
- Ability to problem solve
- Excellent telephone communication skills
- Strong computer skills especially Microsoft, Windows, Excel
- Mature coping skills
- Must complete a background check before beginning employment
