Job Summary
PACE RN role providing direct nursing care to geriatric patients across outpatient and community settings within a PACE program. Responsibilities include performing initial and ongoing patient assessments, developing individualized care plans to promote patient independence, and collaborating with an interdisciplinary care team to deliver high-quality, mission-driven community care. Schedule is Monday–Friday, 8:00 AM–4:00 PM with no weekends. Requires ability to work in both clinic and community/home settings and to support safe community living for older adults. This role emphasizes geriatric-focused care, interdisciplinary collaboration, and autonomy in nursing practice.
Required Qualifications
- Active Massachusetts RN license REQUIRED
- Minimum 2 years recent RN experience REQUIRED
- Comfortable working in both clinic and community/home settings REQUIRED
- Strong assessment and care planning skills REQUIRED
- Team-based care approach REQUIRED
