Job Summary
Provide direct supervision, training, and performance evaluation for physical therapists, occupational therapists, rehab aides, and activity instructors across all PACE sites. Ensure effective staffing, scheduling, and coverage while managing activity instructor contracting and vendor relationships. Lead quality improvement initiatives, participant survey administration, and emergency preparedness training for staff and community protocols. Develop policies for rehabilitation service delivery, coordinate care plans with primary care providers and interdisciplinary teams, and oversee equipment and space utilization. Monitor clinical metrics, resolve grievances, and ensure compliance with CMS, DHCS, and regulatory audits. Prepare for and participate in regulatory audits, implement corrective actions, and maintain accurate documentation in medical records. Utilize Epic as a content and workflow expert to resolve issues and propose solutions. Travel to other PACE sites as needed for supervision, training, or audits.
Required Qualifications
- Bachelor's degree or higher in Physical Therapy, Occupational Therapy, Rehabilitation Sciences, Therapeutic Recreation/Recreational Therapy, or another health related field
- Currently licensed to practice as a Physical Therapist or Occupational Therapist in California, or certified as a Therapeutic Recreation Specialist (CTRS)
- Minimum of two years supervisory or team lead experience in a healthcare setting
- Minimum of two years recent clinical experience in rehabilitation services
- Demonstrated skills in supervision, leadership, and communication and in fostering an effective team environment
- Minimum one year of experience working with older adults and their families in a healthcare or community agency setting
- Ability to travel between PACE sites as needed
- Current BLS and First Aid
- Must be able to fluently speak, read and write English
- This is an FLSA Exempt position
- This is not an OSHA high risk position
- This is a Full Time position
Desired Qualifications
- prior management of multidisciplinary rehabilitation teams strongly preferred
- Minimum one year of experience working with older adults and their families in a healthcare or community agency setting is preferred
- Fluency in other languages is an asset
