Job Summary
Oversee daily center operations and interdisciplinary team functioning to ensure appropriate staffing, effective workflows, and alignment with PACE standards. Provide supervision and mentorship to site-based managers and licensed clinical personnel, including interviewing, hiring, training, and performance evaluation. Partner with the Chief of PACE and Chief Medical Officer to execute operational and clinical objectives while ensuring regulatory compliance with CMS, DHCS, and federal requirements. Monitor site-level performance metrics related to access, utilization, and quality outcomes to support continuous improvement initiatives. Serve as the on-site liaison between center staff, support services, and leadership to promote coordinated care delivery and timely resolution of concerns. Maintain readiness for audits, surveys, and program reviews through documentation and staff preparedness. Requires a current California RN license, five years supervising clinical staff, and bilingual English/Spanish preferred. Full-time role with flexible scheduling including evenings and weekends; bi-weekly pay of $5,192.31 - $5,724.52.
Required Qualifications
- Graduation from an accredited school of professional nursing
- Current, active, and unencumbered California Registered Nurse (RN) license
- Minimum of five (5) years of experience supervising licensed clinical staff in a healthcare setting
- Current BLS CPR card certified by the American Heart Association
- A minimum of one year continuous experience working with frail or elderly populations
- Possession of current, valid, California Driver's License (Class C)
- Ability to pass a criminal history background check and drug screen upon hire
- Annual health examination
- Annual Tuberculosis skin test clearance or chest x-ray
- Proof of immunity to MMR, Varicella, and Hepatitis B
- Proof of Tdap vaccine
- During current flu season, proof of influenza vaccine or a signed declination form
- Ability to lift up to 30 pounds
- Clear hearing
- Adequate vision to read screens and documents, with or without corrective lenses
- Manual dexterity
- Ability to perform a range of physical movements, including reaching, bending, squatting, sitting, standing, stooping, kneeling, twisting, and performing fine motor tasks
- Must be willing to have a flexible work schedule that may include evenings/weekends
- Must be willing to travel as needed
Desired Qualifications
- A Master's degree in nursing or a health-related field
- A registered nurse with a baccalaureate or higher degree in nursing or a related health field and a minimum of three (3) years of experience within the last five (5) years in a home health agency, geriatric care, primary care clinic, or health facility
- A registered nurse with a minimum of four (4) years of nursing experience within the last five (5) years in a home health agency, primary care clinic, or health facility
- Bilingual English/Spanish
