Job Summary
Collect and track real-time data for SD PACE Quality Assurance and Performance Improvement (QAPI), documenting incidents, grievances, and appeals in compliance with CMS, DHCS, and accrediting body regulations. Analyze QAPI datasets to identify performance risks and recommend improvements, leading new projects using Plan-Do-Study-Act processes. Coordinate interdisciplinary team meetings, conduct care plan audits, and facilitate participant care transitions to hospitals or skilled nursing facilities. Prepare routine and special reports for State and Federal regulators, including materials for CMS quarterly calls and audits. This role supports the IDT and Center Managers in maintaining program-wide compliance standards and improving care quality.
Required Qualifications
- Associate's degree or higher in a health-related field
- 1-3 years of relevant work experience
- Experience working in quality or compliance in healthcare
- Familiarity with quality improvement processes
- Experience as a PACE IDT member
- Excellent written and verbal communication skills
- Ability to maintain accurate records
- Active listening ability
- Knowledge of adult learning modes
- Proficiency in Microsoft Office Suite (Word, PowerPoint, Excel, Outlook, Visio)
- Advanced computer skills
