Job Summary

Provide primary care services including diagnosis, treatment of chronic and acute illness, and preventative care for frail, elderly individuals. Conduct health assessments and evaluate patient needs across the clinic, PACE Day Center, and patient homes. Collaborate with the interdisciplinary team to develop comprehensive care plans and make timely referrals to contracted community providers. Maintain a full-time Monday-Friday schedule from 8:00a-5:00p with a valid Colorado driver's license and reliable transportation. This role supports the mission of helping seniors maintain independence and live safely in their homes through coordinated whole-person care.

Required Qualifications

  • Licensed M.D. or D.O. in the state of Colorado
  • Controlled substance license with current DEA license number or ability to obtain
  • Eligible or currently possesses Medicare/Medicaid UPIN number
  • Valid Colorado driver's license, adequate automobile insurance and access to reliable transportation maintained throughout employment
  • Strong clinical reasoning, diagnostic, and medical decision-making skills
  • Knowledge of geriatric medicine, chronic disease management, preventative care and evidence-based practices
  • Ability to lead and collaborate effectively

Desired Qualifications

  • Board Certification or eligibility in Family Medicine, Internal Medicine, or another related specialty preferred
  • Three (3) years of physician experience preferred
  • Experience working with older adults, medically complex populations or multiple chronic conditions preferred
  • Experience in geriatrics, primary care, hospice, palliative care, home-based care, or PACE preferred
