Job Summary
The PTA implements the physical therapy program under the Physical Therapist, delivering therapies to geriatric patients enrolled in PACE, including therapeutic exercise, neuromuscular reeducation, gait training, and use of modalities (heat, ultrasound, electrical stimulation) to restore function and maintain independence. Responsibilities include developing and modifying individualized plans of care, documenting treatments, guiding patients and caregivers in adaptive devices and self-care activities, evaluating treatment effectiveness, ensuring regulatory compliance, and assisting with equipment and supplies. The role requires collaboration with a multi-disciplinary team, adherence to department policies, and commitment to long-term community-based care. Preferred bilingual English/Spanish communication and knowledge of relevant regulations (PACE, CMS, DHCS) and aging acts.
Required Qualifications
- Graduate of an accredited Physical Therapy Program culminating in an associate degree in Physical Therapy
- License as a Physical Therapist Assistant in CA
- BLS/CPR Certification
- 2 years field experience with geriatric population
- Bi-lingual English/Spanish strongly preferred
