PACE Personal Care Worker

Job Summary

Support physicians and nurses by taking vitals, assisting in medical examinations, and providing translation services. Assist participants with activities of daily living including bathing, dressing, grooming, and toileting. Monitor and assist with meals, maintain clean day rooms and kitchens, and accompany enrollees to appointments. Perform laundry, assist drivers with loading and unloading, and maintain participants' medical records per VCC's charting and reporting requirements. Note changes in physical or emotional health and communicate to appropriate staff. Follow all PACE policies, OSHA safety guidelines, and infection control procedures while maintaining confidentiality. Participate in interdisciplinary team activities and quality improvement initiatives.

Required Qualifications

  • High school graduate or equivalent
  • CPR Certification (including BLS Child, Infant, Adult)
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public
  • Basic knowledge in working with frail and elderly to support Activities of Daily Living
  • Skilled in identifying and recommending problem resolution
  • Ability to work independently with minimum supervision
  • Ability to communicate clearly and effectively
  • Ability to work effectively with cultural, economically, and educationally, diverse populations

Desired Qualifications

  • Minimum of one years of experience in working with the frail or elderly population preferred
  • Bilingual English / Armenian
  • Experience/familiarity with Practice Management and Electronic Medical Records systems, NextGen preferred
