Job Summary
Personal Care Assistant at Central Valley PACE Center in Modesto, CA, delivering basic personal care and activities of daily living support, documenting treatments and observations in EMR, delivering DME to participants’ homes, and collaborating with the licensed nurse and nursing staff. Role involves communicating with participants, families, and staff, participating in participant care planning, and maintaining professional interactions in a geriatric care setting. Schedule is Monday–Friday, 8:00am–5:00pm with occasional weekend flexibility; requires up to 30 lb lifting capability and maintaining attention to safety and regulatory compliance.
Required Qualifications
- High School Diploma or equivalent
- BLS CPR Certification
- Prior personal care assistance experience or ability to pass internal competencies within six months
- Valid California Driver’s License with acceptable driving record and vehicle insurance
