Job Summary
Provides personal care and supportive services to PACE participants in participants' homes, with collaboration from the Interdisciplinary Team (IDT) and according to the plan of care. Services are primarily delivered in participants’ homes but may also occur at the PACE center or other care settings. Assists with activities of daily living, maintains a clean and safe environment, and observes/reports changes in participant condition to clinical staff. Requires a high school diploma or GED, CPR certification, 18+ years old, and ideally one year of home health aide experience; familiarity with electronic medical records preferred; must be a licensed driver. Emphasizes compassionate care and effective communication with adults and the elderly; supports equal employment opportunity.
Required Qualifications
- High school diploma or GED required
- Must be at least 18 years of age
- One year of home health aide experience preferred
- Must maintain CPR certification
- Knowledge of safety and infection control requirements for healthcare facilities
- Knowledge of medical equipment use and maintenance
- Must be a licensed driver
