Job Summary

Screen and assess member functional status, including gait, balance, and home safety evaluations, to develop and implement physical therapy plans of care. Provide direct guidance to Patient Care Aides and supervise Rehabilitation Assistants and Physical Therapy Assistants, while conducting group education for members and training staff on body mechanics and assistive devices. Collaborate with the interdisciplinary team on care planning, discharge planning, and service allocation, and advocate for community-based services for the frail elderly population.

Required Qualifications

  • Must have knowledge and understanding of and be sensitive to area minority groups and their socio-economic backgrounds/cultures

  • Advanced knowledge and training sufficient to obtain and maintain licensure in the state of California
  • Bachelor's Degree in Physical Therapy from an accredited program
  • Current State PT license
  • Valid California driver's license
  • A minimum total of one year of personal, clinical/internship, and or employment experience providing care or services for adults with disabilities or frail elderly population
  • Working knowledge of physical, mental and social needs of disabled and older adults
  • Effective skills in assessment of functional needs of disabled and older adults
  • Effective written and oral communication skills
  • Productive as both an independent and interdisciplinary team contributor
  • Effective organizational skills
  • Demonstrates necessary skills and knowledge as needed for position-specific Competency Assessment Profile

Desired Qualifications

  • Ability to communicate effectively, both verbally and in writing in both English and Spanish preferred
  • Experience/familiarity with PACE program
  • Experience/familiarity with Electronic Medical Records systems
  • Bilingual (English/Spanish) preferred
  • Able to quickly build and maintain rapport with patients and providers of differing backgrounds
