Job Summary
Provide direct nursing care and supportive clinical services to participants across the PACE Center, home, hospital, nursing home, and assisted living settings. Administer medications, observe and report changes in condition, document care, coordinate care, and communicate with the interdisciplinary team under the supervision of the Registered Nurse and PACE leadership. Promote participant safety, continuity of care, regulatory compliance, and a high-quality care experience for participants and their caregivers. Requires current LPN licensure, at least one year of experience with the frail and elderly, and the ability to navigate diverse physical environments. Monday-Friday 8:00am-4:30pm with on-call rotation.
Required Qualifications
- Graduate of an accredited school of nursing or vocational nurse program
- Current LPN licensure through the State Board of Nursing
- At least one year of experience working with the frail and elderly
- Ability to use equipment with visual and auditory mechanisms
- Ability to physically access homes, hospitals, nursing homes, and assisted living facilities, including navigating stairs, narrow spaces, and environments without air conditioning
- Strong communication skills, including face-to-face, telephone, and computer skills
- Ability to interact successfully with participants, families, and other agencies to present a positive and professional image
- Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
- Knowledge of the principles and practice of primary medical care
- Monday- Friday 8:00am-4:30pm with on-call rotation
