Job Summary

Directly provides services to frail elders in their homes under supervision of the Community Services Department. Responsibilities include personal care and homemaker tasks, assistance with eating and toileting, support with ambulation, transporting clients to medical appointments, and assisting with activities at centers or clinics. Performs tasks such as colostomy care, bathing, peri-care, mouth and skin care, and light housekeeping; may assist with meal preparation and special diets. Maintains client safety and clean work areas, reports on participant status to administrative staff, and may participate in training and orientation of staff/volunteers. Flexible schedule including weekends; serves clients across San Francisco districts; works as part of an interdisciplinary health-care team and may assist with program activities or rehabilitation exercises. Requires prior experience with older adults and the ability to communicate effectively in English; preferred CNA certification and multilingual abilities (Spanish, Russian, Vietnamese, Korean, Chinese).

Required Qualifications

  • At least one year working with older adults.
  • Ability to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
