Job Summary

Schedule and reconcile all home care services, creating participant schedules based on care plans and staff availability. Maintain staffing coverage for leave, communicate with contract agencies, and assist with vendor invoice reconciliation and EMR delivery. Coordinate DME, deliver equipment, and assist with semiannual assessments while contacting providers and clients regarding day-to-day changes. Participate in rotational after-hours and weekend calls for emergencies and referrals. Perform general office duties including reception coverage and data entry. Requires a valid CA driver's license, BLS CPR certification, two years caregiver experience, and bilingual English/Spanish proficiency.

  • Proficiency with Microsoft Suite
  • Proficiency with Google Suite
  • Proficiency with scheduling software
  • Bilingual English / Spanish
  • High school diploma, or equivalent
  • Current BLS CPR Card certified by American Heart Association
  • Minimum two (2) years caregiver experience
  • Minimum one (1) year experience scheduling home care appointments
  • Ability to lift up to 30 pounds
  • Must be able to hear staff on the phone and those who are served in-person
  • Must be able to speak clearly in order to communicate information to clients and staff
  • Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents
  • Must have high manual dexterity
  • Must be able to reach above the shoulder level to work
  • Must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling
  • Must be able to work Monday – Friday
  • Must be able to work 8:00am – 5:00pm
  • Must be available for after hours and weekend calls on a rotational basis
