Job Summary
PACE Home Care Coordinator role focused on coordinating IDT-approved services for SD PACE participants, including meal delivery, DME, and caregiver services; conducts home safety evaluations, enrollment and semi-annual care-plans, and maintains EMR documentation; collaborates with physicians, social workers, therapists and caregivers; advocates for participant needs with a person-centered approach; travels to participant homes and SYHC sites; supports SD PACE nurses and attends interdisciplinary team meetings; adheres to infection control and safety protocols; requires CNA and HHA certifications and CA driver’s license with ongoing background checks.
Required Qualifications

- Valid CA driver license and vehicle insurance
- CPR and First Aid (BLS) certification
