Job Summary
Coordinate IDT-approved meal delivery, Durable Medical Equipment (DME), and caregiver services for SD PACE participants; conduct enrollment and ongoing home safety evaluations; document and track actions in EMR per DHCS and CMS regulations; inform IDT of service gaps and coordinate care plans; collaborate with physicians, social workers, therapists, and other healthcare professionals to ensure coordinated, person-centered care; provide support to SD PACE nurses and liaise with caregiver agencies and DME suppliers; conduct monthly visits for homebound/RCFE participants and travel to participant homes and SYHC/PACE sites as required; maintain confidentiality, adhere to infectious disease precautions, and ensure timely reporting and documentation; require CNA and HHA certifications, CPR/First Aid, and strong communication and organizational skills to manage geriatric participant care within scope of Home Health Aide practice.
Required Qualifications
- CNA Certification
- HHA Certification
- Valid CA driver’s license and vehicle insurance
- CPR and First Aid (BLS) certification from the American Heart Association
