Job Summary
Transport participants, medications, meals, and medical supplies between homes, clinics, and the PACE center while adhering to scheduled routes and regulatory compliance. Safely assist passengers in boarding and disembarking, manage vehicle maintenance logs, and promptly report safety or behavioral issues to the Interdisciplinary Team. Maintain First Aid kits, ensure vehicle mechanical condition, and utilize Epic as a workflow expert to resolve service issues. Establish positive rapport with participants, families, and care team members while participating in route planning and IDT meetings. Maintain accurate daily records and support vehicle maintenance appointments.
Required Qualifications
- High school diploma or equivalent
- 1 year of commercial driving experience
- Valid CA Class C Driver's license
- Class B license
- No violations within the last 5 years
- Basic Life Support and First Aid certification
- Ability to operate mid to large size vehicle
- General automobile mechanism knowledge
- Ability to work flexible hours
- Ability to work Monday through Friday
- Ability to work occasional Saturday
- Ability to follow written and verbal directions
- Ability to effectively communicate with staff and patients/participants
- Ability to work independently
- Ability to work as part of a team
- Fluency in English (speak & listen)
- Minimum reading and writing ability in English
Desired Qualifications
- Class B license (no violations within the last 5 years)
- Knowledge of assisting patients or persons with special-needs populations
- Knowledge of individuals with mobility limitations
- Knowledge of individuals with medical conditions
- At least one year of experience transporting passengers
- Knowledge of proper body mechanics for assisting seniors and persons with mobility limitations
- One-year experience with the elderly
- Knowledge of working with special needs populations
- Knowledge of individuals with Dementia
- Fluency in Chinese
- Fluency in Vietnamese
- Fluency in Spanish
- Fluency in other languages
