Job Summary
PACE Community Services Nurse responsible for assessing level-of-care eligibility for prospective PACE enrollees, conducting in-home nursing assessments, developing individualized in-home Care Plans in collaboration with the IDT and Home Care Supervisor, and delivering community-based care (in-person, telehealth, or by phone). Requires coordination with the Interdisciplinary Team (IDT), completion of level-of-care documentation for DHCS, and ensuring accurate medical records. Role includes leadership within the department, staff training/competency, and potential after-hours on-call coverage; Epic experience and home-care best-practice knowledge emphasized. Fluent English required; Cantonese and/or Vietnamese skills are an asset; California RN license and a valid driver license are required.
Required Qualifications
- Valid unrestricted California RN license
- Graduate of an accredited school of Nursing
- Current BLS and First Aid required
- Valid CA driver license (no violations within the last 5 years)
