Job Summary
PACE Community Services Nurse responsible for assessing level-of-care eligibility for prospective PACE enrollees, developing in-home care plans with the Interdisciplinary Team (IDT), conducting comprehensive nursing assessments and home evaluations, coordinating with the Home Care Supervisor to implement care plan services, ensuring documentation in medical records, providing training and competency checks for caregiving staff, delivering health education to family caregivers, and utilizing Epic for workflow optimization. Role includes delivering in-person, telehealth, and telephone care, and potential after-hours on-call support; requires English fluency and Cantonese and/or Vietnamese as an asset.
Required Qualifications
- Valid unrestricted California RN license
- Graduate of an accredited school of Nursing
- Current BLS and First Aid
- Valid CA driver license (no violations within the last 5 years)
