Job Summary
Deliver personal care and related services to PACE participants in the clinic, day center, homes, long-term care facilities, or hospitals under the direction of the PACE Nursing Staff. Collaborate with the Interdisciplinary Team and adhere to the plan of care for assigned seniors. Maintain CPR certification and utilize electronic medical records while ensuring safety, infection control, and proper medical equipment maintenance. Hold a valid Kentucky driver's license with proof of insurance and demonstrate clear communication skills. Requires State Registered Nurse Aide certification, one year of home health or nursing assistant experience, and familiarity with frail elderly populations.
Required Qualifications
- State Registered Nurse Aide (SRNA) certification required in the state of Kentucky
- Must maintain CPR certification
- Must have a valid driver's license in accordance to policy with proof of automobile insurance required
- Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire
Desired Qualifications
- High school diploma preferred
- One year home health aide or nursing assistant experience preferred
- Use of technology-based tools, including electronic medical records, preferred
- Knowledge of safety and infection control requirements for healthcare facilities
- Knowledge of medical equipment use and maintenance
- Ability to communicate clearly and effectively
