PA - Philadelphia - RN Case Manager

Manage a diverse caseload ensuring quality patient care and efficient care coordination while providing coaching and oversight to Certified Nursing Assistants. Develop individualized care plans based on patient assessments and coordinate with Primary Care Physicians, therapy providers, and internal support teams to ensure comprehensive care. Maintain accurate electronic charting with a minimum 24-hour turnaround time and establish effective communication channels to escalate issues to Clinical Leads. Provide education and support to patients and families regarding health care needs and treatment plans. Participate in on-call rotation including approximately one weekend per month, holiday rotation, and possible phone triage night coverage. Monitor patient readmission rates and work to improve extended visit ranges.
