Job Summary

Independently evaluate, diagnose, and manage patients with advanced heart failure in the Heart Failure Bridge Clinic by developing individualized treatment plans, optimizing guideline-directed medical therapy, and initiating IV diuretic interventions. Coordinate transitions between inpatient and outpatient care while educating patients and families on management strategies. Collaborate with physicians, nurses, pharmacists, and case managers to deliver comprehensive, patient-centered care and support evidence-based quality improvement initiatives. Work alongside Advanced Heart Failure cardiologists in a specialized outpatient setting with four 10-hour weekday hours, no weekends or holidays, and no call responsibilities.

Required Qualifications

  • Graduate of an accredited Physician Assistant program
  • NCCPA certification
  • Current Maryland Physician Assistant license (or eligibility)
  • Minimum of two (2) years of recent cardiology experience as a practicing Physician Assistant
  • Experience managing patients with cardiovascular disease in an in-patient and/or outpatient setting
  • Strong clinical judgment, critical thinking, and decision-making skills
  • Excellent communication and interpersonal skills with a commitment to collaborative, patient-centered care
