Job Summary
Independently evaluate, diagnose, and manage patients with advanced heart failure in the Heart Failure Bridge Clinic by developing individualized treatment plans and optimizing guideline-directed medical therapy. Assess volume status, initiate IV diuretic interventions, and coordinate transitions between inpatient and outpatient care to reduce hospital readmissions. Educate patients and families on management strategies while collaborating with physicians, nurses, pharmacists, and multidisciplinary team members to deliver comprehensive, patient-centered care. Support evidence-based practice, quality improvement initiatives, and established clinical protocols, including documenting and billing for professional services. This role offers a four 10-hour weekday schedule with no weekends, holidays, or call responsibilities, allowing for specialized outpatient practice with high autonomy within a collaborative academic environment.
Required Qualifications
- Graduate of an accredited Physician Assistant program
- NCCPA certification
- Current Maryland Physician Assistant license (or eligibility)
- Minimum of two (2) years of recent cardiology experience as a practicing Physician Assistant
- Experience managing patients with cardiovascular disease in an in-patient and/or outpatient setting
- Strong clinical judgment, critical thinking, and decision-making skills
- Excellent communication and interpersonal skills with a commitment to collaborative, patient-centered care
