Outreach Worker - TEMP Position

Conduct progressive outreach attempts weekly via telephone, face-to-face visits, and digital channels to locate and engage individuals with mental health, substance use, and chronic medical conditions. Perform enrollment activities by collecting eligibility documentation, scheduling intakes, and uploading forms to the EHR within 48 hours of member registration. Manage a caseload of up to two members, coordinate with the Outreach Director and supervisors for accurate status reporting, and complete all required billing and monthly Medicaid/MCO status monitoring. Maintain detailed medical records, utilize programmatic data entry systems, and adhere to agency and regulatory policies while attending periodic trainings and supervision.
