Job Summary
Conduct community-based outreach activities including home visits, provider visits, telephone calls, and correspondence to locate and enroll Health Home eligible persons. Confirm patient information and coverage via EPACES, then document all activities in the Health Home EMR and submit a daily Time Log to the Intake Coordinator. Complete intake packets, Demographics Forms, and screening forms as assigned, while responding to calls regarding patients in hospital Emergency Rooms. Work flexible or non-traditional hours from 2pm to 10pm at an hourly rate of $17.54.
Required Qualifications
- High School Diploma or GED
- Computer literacy including proficiency in Microsoft Office Suite and EHR
- Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds
- Knowledge of community resources
- Sensitivity towards persons with chronic illnesses and mental health related conditions
- Willingness to conduct field work in the community to connect with clients
- Available to travel throughout the required catchment area as needed by navigating the public transportation systems
- Available to work a flexible schedule or non-traditional work hours as needed in response to program and staff needs
