Job Summary
Conduct non-clinical Treatment Plan interventions for Persons Served in community, group living, or supported independent environments as directed by Team LPHA Clinicians. Deliver skills-building support across daily living, medication administration, symptom reduction, employment, and socialization while attending medical and psychiatric appointments. Provide crisis intervention, safety planning, and resource navigation using trauma-informed, recovery-oriented, and culturally competent approaches. Maintain professional relationships with community providers and ensure thorough documentation of needs and services. Transport Persons Served using a Vinfen van or personal vehicle with valid license and insurance. Maintain CPR, First Aid, and MAP certifications within required timelines.
Required Qualifications
- A high school diploma or equivalent
- Must have access to an operational and insured vehicle and be willing to use it to transport PS
- Must possess and maintain adequate insurance
- Must maintain a safe driving record
- A valid driver's license
- Must be at least 21 years of age
- Must have maintained a valid US driver's license for at least six months
- Must be able to pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb
- Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift
- Ability to operate a computer and other office equipment such as a calculator, copier, and printer
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to speak, hear, and communicate with PS, staff, and external representatives
- Specific vision abilities including close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus
- Ability to remain in a stationary position 50% of the time as needed
- Ability to bend, reach, file, sit, stand, and move around the facility
- Ability to assist PS with tasks of daily living
- CPR required within first two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
Desired Qualifications
- A Bachelor's degree in human services field
- One to two years related experience working with people with psychiatric disability and/or co-occurring disorders
- Bilingual/bicultural applicants
- Applicants with lived experience of psychiatric conditions
