Job Summary
Perform outreach activities for the HealthySteps program in Family Medicine, including administering assessments to children and families aged 0-3, conducting community resource mapping, linking clients to services, and promoting program awareness. Act as a liaison between HealthySteps at CHI and the community, maintain accurate activity records and daily postings, support marketing efforts, ensure timely documentation and billing, and meet monthly goals while supporting follow-up with consumers. Requires strong communication and interpersonal skills, and preferably bilingual abilities; CPR certification and a Florida driver’s license are required.
Required Qualifications
- High School Graduate or GED Equivalent
- CPR certification from the American Heart Association
- Valid Florida Driver's License
