Job Summary
Outreach Support and Resource Coordinator role in Peterborough, ON (Full-time, 35 hours/week, 18-month contract) focused on connecting care partners and individuals living with dementia to supports, coordinating programs, and providing information and practical assistance. Responsibilities include building supportive relationships, guiding families to community services, coordinating initiatives like Project Lifesaver and Finding Your Way Ontario, coordinating counselling volunteers, backing up care partner support groups, and providing administrative support for counselling operations. Requires travel throughout the four-county region, a valid driver’s license, and a Vulnerable Sector Check. Education in Social Services/Social Work/Psychology/Gerontology/Human Services/Nursing, 2-3 years in healthcare or social services, and strong interpersonal, organizational, and communication skills are sought. Compensation ranges from $22.00-$27.00 per hour with benefits, vacation, sick days, and flexible scheduling considerations.
Required Qualifications
- Diploma or degree in Social Services, Social Work, Psychology, Gerontology, Human Services, Nursing, or related field
- 2-3 years' experience working in healthcare, social services, community services
- Experience working with older adults, people living with dementia and their care partners
- Experience in assessment, care planning, and service coordination
- Strong knowledge of dementia care and person-centered supportive counselling practices
- Valid G Class driver’s license and access to a reliable, insured vehicle
- Vulnerable Sector Police Check (within the last 2 months)
