Job Summary
Outreach Specialist to join Pathways Vermont’s new team supporting Vermonters experiencing homelessness to find permanent housing and access community resources. Responsibilities include engaging in assertive outreach, service coordination, advocacy, and other supports; building rapport with individuals in motels, shelters, and camping areas; facilitating telehealth or in-person meetings between the team’s mental health counselor and program participants to complete mental health and substance use screening; ensuring households are captured in HMIS; making referrals to community resources and assisting with housing subsidies and rental-unit applications; documenting service provision in electronic health records; maintaining records and reporting; and supporting client-driven referrals and participation in peer services and related activities.Qualifications emphasize a Bachelor's degree or comparable experience and 3-5 years relevant experience, strong computer/database skills, flexibility with hours, valid driver's license and willingness to travel statewide, and excellent communication. Attendance at annual staff retreats is expected.
Required Qualifications
- Bachelor’s Degree or comparable work experience (preferably with persons who have experienced homelessness or have a psychiatric disability) and at least 3-5 years relevant experience
- Strong skills in database entry and computer programs (including Google Apps, Internet browsers, electronic medical records, etc.)
- Must be able to work in a fast-paced environment as well as possess strong problem- solving skills, work ethic, and computer proficiency
- Must be able to work flexible hours, occasional long days
- Valid driver’s license, insurance, and reliable vehicle required
- Ability to travel statewide
- Excellent verbal and written communication
