Job Summary
Conduct targeted outreach to local community agencies, health fairs, and specialty practices to increase awareness of My Healthy VT Self-Management programs. Provide one-on-one health coaching to patients, support behavior change through goal setting, and administer pre- and post-pilot assessments. Host workshops for chronic disease prevention and manage electronic health record panel data while collaborating with physicians and nurses to ensure programming reaches patients. This role requires an Associate Degree and NBC-HWC credential, with a salary range of $24.28-$31.97. The position is full-time (80 hours bi-weekly) from 8 am to 4:30 pm, located at Northwestern Medical Center in Vermont.
Required Qualifications
- Associate Degree, or High School degree with 3 years of relevant experience
- NBC-HWC health coach credential
- Administrative and health coaching experience
- At least two years working in health promotion, health education, health administration, nutrition, and/or self-management or other health promoting programs
- Strong written and verbal communication skills
- Strong sense of organizational commitment, customer orientation, performance focus, and self-management
- Proficiency in Microsoft Office applications such as Outlook, Excel, and Word
- Proficiency in Web meeting platforms such as Microsoft Teams
- Grant management skills
- Use tact, diplomacy, and professionalism at all times
- Ability to work independently and meet stated goals
- Commitment to excellent and culturally responsive customer service and communication
- Proven ability to work collaboratively with individuals and groups
- Ability to travel to multiple community sites in one day
- Means of private transportation is required

- Demonstrate thorough understanding of social determinants of health
