Job Summary
Conduct intensive community outreach to identify and assist homeless and at-risk veterans with accessing services through the Military Resource Center based on the psychosocial rehabilitation model. Administer the SSVF Assessment tool to determine eligibility while maintaining an outreach log and providing bi-weekly reporting to intake staff. Make regular visits to shelters, emergency rooms, jails, and social service agencies to establish rapport with referral sources and respond to community-based requests for assistance. Coordinate initial screening, case management, and participation in stand-downs to meet monthly enrollment outcomes, while helping veterans locate housing, food, healthcare, and benefits. Serve as a spokesperson at public events and meetings to build local partnerships and ensure seamless service delivery. Maintain all compliance records and enter billing documentation into LAHSA's HMIS system within 24 hours of service delivery.
Required Qualifications
- High School diploma or GED
- One year of providing psychiatric rehabilitation services to a disabled population/low income and/or homeless population
- Knowledge of health care, social service resource systems and self-help intervention strategies
- Crisis intervention and conflict resolution skills including use of motivational interviewing, harm reduction approach, and trauma-informed care
- Strong oral/written communication and listening skills
- Team-oriented

- Ability to empathize and relate positively to a wide variety of people
- Dedication to fulfilling the demands of the position
- Experience working in diverse settings with people across all socio-economic spectrums
- Proficient typing and computer skills, including Microsoft Office Suite (Outlook, Word and Excel)

- Flexible work schedule to include some evenings or weekends when necessary
- Driving with current class C driver's license
- A vehicle for transportation
- Proof of valid automobile insurance
- The ability to be insured by MHALA's liability insurance
- TB clearance
- Re-testing every year
- Employment verification directly from three (3) past employers within the most recent 7-year period
