Outreach Specialist

Job Summary
Conduct targeted patient outreach to identified individuals with gaps in chronic disease management and preventive care across Newport, Oxford, St. Clair Shores, Novi, and portions of Dearborn. Utilize EMR systems, registries, and health maintenance data to validate gaps, initiate contact via portal messages, phone calls, or letters, and coordinate access to services and wellness support. Maintain tracking of outreach and gap closure while collaborating with practice staff and the Practice Transformation Team to address quality care improvements and documentation issues. Perform quarterly quality measure audits, complete scheduled eye exams at ambulatory clinics, and investigate community resources to support clinic development. Work on-site at up to 15 clinics Monday through Friday from 8:00 a.m. to 4:30 p.m. as part of Corewell Health's mission to improve health outcomes and close care gaps.
Required Qualifications
- High School Diploma or equivalent
- Minimum of 2 years of experience as a medical assistant in a physician clinical practice, a clinical nursing program, or in a population health related role

- EPIC experience
- Completion of an accredited Medical Assistant program
