Job Summary
Outreach Specialist supports outreach, engagement and linkage work by collaborating with homeless outreach teams, including conducting outreach at clients’ residences or locations of choice and providing behavioral health services. Responsibilities include facilitating transportation to services, linking clients to housing, food, clothing, and financial assistance, training on public transportation, advocating for clients, completing required paperwork (e.g., progress notes, HMIS), and acting as a Mandated Reporter while safeguarding PHI under HIPAA. The role requires strong communication, empathy, and collaboration with service providers, law enforcement, and the community to support clients in achieving their goals.
Required Qualifications
- High School Diploma or GED
- Valid California Driver’s License and current vehicle insurance
- Must have at least 3 years of driving experience
- Ability to pass FBI and/or DOJ background screenings
