Job Summary
Provide direct supervision of care coordinators in Sonoma, Orange, and San Bernardino Counties while actively executing daily tasks like patient outreach, care coordination, and provider communication. Lead and support the team through coaching, mentoring, and performance management to ensure competency and compliance. Collaborate with Operations and Learning & Development teams on hiring, training, and deployment, while fostering PCP partnerships and building community connections through in-person and virtual meetings. Review data dashboards to manage team performance against goals and identify improvement opportunities. This role requires up to 30% in-person travel and resides in one of the specified counties.
Required Qualifications
- Deep commitment to improving healthcare for underserved populations
- History of supervising teams of care coordinators, patient navigators, customer service, outreach specialists or similar roles
- A leader with cultural and personal humility
- Strong understanding of Medicaid/MCOs and the barriers to healthcare access and quality among diverse, underserved populations
- Keen eye for detail and proven track record of effectively monitoring and optimizing metrics, leveraging Excel skills or other tools to analyze data and drive continuous improvement initiatives
- Demonstrated ability to collaborate and communicate effectively in a multidisciplinary team setting
- Experience with healthcare data and/or Electronic Health Records
- Ability to travel across your local county to help establish new teams and support existing teams
- Reside in one of these counties: Sonoma/Orange/San Bernardino
- COVID vaccination status attestation
Desired Qualifications
- Bachelor's degree in health administration, health management, public health, business administration, or related field
- Long time resident of one of the 3 identified counties and knowledgeable of MCO and community resources
- Multilingual skills
