Job Summary
Outreach Navigator provides outreach to individuals with serious mental illness and/or eligible MHBG individuals, identifying and reducing barriers to services, connecting clients with needed resources (including COVID-19 PPE supplies and education), and coordinating wrap-around supportive services. The role involves engaging SMI/behavioral health clients pre/post incarceration or after a Crisis Mobile Team activation, mapping next steps for service access, and documenting patient interactions per AAC, ADHS/DBHS Provider Manual, CARF, and program policies as part of a multidisciplinary team.
