Job Summary
Provide clinical therapy services (individual, family, group) in community-based and home settings for individuals recently discharged from hospitalizations or facing barriers to care. Engage and retain clients using Evidence-Based Practices, crisis intervention, and telehealth technologies while collaborating with peer advocates, case managers, and medical providers. Maintain a clinical caseload, complete accurate documentation in EMR tools, and participate in weekend rotation hours as scheduled. Support program development for mobile and home-based services and assist with quality improvement efforts. Work across CCBHC sites in Middletown, Newburgh, and surrounding communities.
Required Qualifications
- Licensed Master Social Worker (LMSW) or LP-MHC or MFT in New York State
- Master's degree in Social Work or Mental Health Counseling
- Experience with crisis intervention and working with diverse populations
- Valid NYS driver's license and reliable transportation for community-based work
- Must be capable to access all rooms in a 2-story home
- Able to work in open space floor plan
- Must be capable to move throughout work day
- Occasional lifting of > 25+ pounds
Desired Qualifications
- Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC) or Licensed Marriage and Family Therapist (LMFT)
- Experience with substance use disorders; CASAC credential is a plus
- Experience working with individuals diagnosed with intellectual/developmental disabilities
- Prior work experience in mobile, home-based, or outreach-focused behavioral health programs
