Job Summary
Outreach & Wellness Support Worker provides community-based outreach, advocacy, and systems navigation for Yuułuʔiłʔatḥ citizens and families, focusing on Port Alberni and surrounding communities. Key responsibilities include building trusting relationships through in-person engagement, assisting with access to primary health care, mental health and housing supports, navigating provincial/federal systems (e.g., First Nations Health Authority, Island Health, MCFD), accompanying members to appointments, coordinating wraparound supports with health and social services staff, and advocating for culturally safe and trauma-informed care. The role also supports restorative justice and diversion processes in partnership with Elders, families, and justice partners, and engages with Indigenous organizations and community supports to strengthen safety, stability, and wellness. Required qualifications include a Diploma or degree in a related field (or equivalent experience), valid BC driver’s license, and a vulnerable‐sector background check. Preferred qualifications include experience in restorative justice, regional knowledge of Port Alberni and West Coast services, trauma-informed practice training, and familiarity with service pathways through First Nations Health Authority, Island Health, and MCFD.
Required Qualifications
- Diploma or degree in Social Work, Human Services, Indigenous Studies, Criminology, Health, or a related field — or an equivalent combination of relevant experience and lived/community knowledge.
- A combination of education and experience in outreach, community support, health/social services, or a related field.
- Demonstrated ability to work with Indigenous individuals and families navigating complex systems.
- Valid Class 5 BC Driver's Licence and access to reliable transportation.
- Satisfactory Criminal Record Check, including Vulnerable Sector screening.
