Job Summary
The Outreach Housing Specialist provides essential coordination and support for individuals and families experiencing homelessness. Key responsibilities include engaging clients for shelter referrals, assisting with housing applications, participating in case management, and maintaining thorough client records. This role requires the ability to conduct interviews to evaluate clients' needs, navigate housing and medical appointments, gather necessary documentation for program eligibility, and support clients in securing permanent housing. A strong knowledge of community resources and the ability to work effectively in diverse, multicultural settings are essential.
Required Qualifications
- 1 Year of outreach experience required
- Two (2) years of related work experience is required
- Knowledge of homeless community resources and community services is required.
- Experience to work with multicultural and diverse population.
- Experience in Rapid Re-Housing program, Permanent Supportive Housing and FQHC program.
