Job Summary
Provide mobile, non-clinical outreach and engagement to high ED utilizers and post-overdose individuals; conduct outreach, follow-ups, triage assessments, transport coordination, warm hand-offs, and liaise with partners to connect clients to behavioral health services. Build rapport with community stakeholders and referral partners, document timely updates, participate in staff trainings, and assist with transportation to intake appointments as part of comprehensive recovery-oriented support.
Required Qualifications
- High School Diploma/GED required
- Bachelor's Degree preferred
- At least 1 year of related outreach or engagement experience
- Knowledge of local community associations or organizations
- Excellent written and verbal communication skills
- Must be highly organized, detail-oriented, and able to multitask in fast-paced settings
- Computer literacy and familiarity with documentation systems
- Must possess a valid driver's license and insurance
- Eligible for Certified Recovery Support Specialist (CRSS) certification within one year of hire
