Job Summary
Outreach Coordinator in the Case Manager – Outreach program focused on Staten Island. Responsibilities include connecting with homeless individuals, completing assessments and referrals (psych evaluations, TB and HIV testing, detox), assisting clients with entitlements, compiling psychosocial histories, counseling toward interim/transitional/permanent housing, completing required paperwork and statistics, responding to community outreach calls, driving and escorting clients to services using the Outreach van, and participating in weekly supervision, case conferences, and staff meetings.
