Job Summary
Conduct outreach to newly assigned and diverse patient populations to establish primary care services, promote preventive screenings, and support population health initiatives. Connect current and prospective patients with NEMS resources while identifying barriers to care and assisting with data collection for quality reporting. Track outreach outcomes related to performance improvement efforts and participate in continuous quality improvement activities. Ensure compliance with all applicable regulations and organizational policies. Fluently speak, read, and write English with required fluency in Chinese (Cantonese and/or Mandarin).
Required Qualifications
- High school diploma
- Minimum of 2 years of relevant work experience
- Associate or Bachelor's degree
- Experience with medical appointment scheduling
- Fluent in Chinese (Cantonese and/or Mandarin)
- Must be able to fluently speak, read and write English
- Ability to follow through with delegated tasks and self-manage in a detail-oriented environment
- Good organization and prioritization skills to manage multiple projects and timelines
- Strong interpersonal skills to collaborate effectively with a broad range of internal and external stakeholders
- Excellent communication (spoken and written) and time management skills
Desired Qualifications
- Experience with Epic EHR scheduling
- Prior experience in a managed care setting or IPA provider organization
- Prior experience with customer service; can build rapport with patients via phone
- Proficient with Microsoft Office and PC based software programs or automated database management systems
- Fluency in other languages
- Associate or Bachelor's degree
