Job Summary
Conduct street outreach in Brockton/Quincy and surrounding areas twice weekly to build relationships with unsheltered individuals and coordinate services for stable housing. Perform intake assessments, create individual service plans, and maintain accurate case records while submitting written outcome reports. Provide crisis intervention, mediation, and advocacy to help guests achieve self-sufficiency, while collaborating with internal teams and external providers to streamline referrals. Participate in strategic planning, policy development, and collaborative meetings as required. Full-time role in Quincy with 10:00am–6:00pm hours (Monday–Friday, with weekend variation); CPR certification required.
Required Qualifications
- Bachelor's degree in a related field or equivalent work experience
- CPR certification
- Valid driver's license
- Good driving record
- Average visual acuity
- Ability to hear average or normal conversations
- Ability to verbally convey detailed information or important instructions or ideas accurately or quickly
- Acceptable overall attendance record
- Consistently at work and on time
- Relied upon regarding task completion and follow-up
- Follows instructions
