Job Summary
Conduct HEN Program intakes at the Bremerton office and Poulsbo site, while establishing new intake locations across Kitsap County to improve client access. Perform case management and crisis intervention for vulnerable clients facing housing and resource barriers, including creating service plans, transporting clients to housing visits, and advocating on their behalf. Coordinate with the Coordinated Entry provider to screen clients and place them in priority pools based on vulnerability. Ensure strict compliance with Consolidated Homeless Grant guidelines by maintaining complete client files, documenting crisis responses, and entering data into HMIS and HVIIS systems. Prepare rent request spreadsheets, manage supply orders, and respond to client inquiries when the Program Manager is unavailable.
Required Qualifications
- Post-secondary education in a social services field
- Minimum of 2-year experience working with individuals and/or families with complex needs
- Understanding of problems homeless people often face, including alcohol and drug abuse, mental illness, domestic violence, and long-term homelessness
- Excellent written and oral communication
- Good office computer skills, with ability to maintain complex client records and update databases including HVIIS
- Commitment to working within the mission, goals and objective of Catholic Community
- Support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services
- Support and contribute to a creative, collaborative and respectful environment that promotes teamwork
- Mental health and/or crisis intervention experience
- Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations
- Must have reliable transportation, valid driver's license, and automobile insurance and ability to transport clients in your personal vehicle
- Must have an acceptable driving record per CCS' driving policy
- Applicant must successfully pass required background checks prior to an offer of employment
